NHS Decisions

Your NHS has been asked to make some big decisions about what healthcare will be available to us in future and how it will be organised.

NHS leaders and councillors are working together on these huge plans (known as STPs - or Sustainability and Transformation plans).

The plans will make changes to your existing services that will affect everyone.

This survey aims to help collect people's views about these important local decisions and pass them to your local planners, to influence the decisions about the future of our NHS, that are going on right now.
