A report produced by the Nuffield Trust and Age UK concludes that there are very few signs that integrated and community care initiatives produce reductions in costs or emergency admissions to hospital.
The NHS long-term plan is focused on the use of integrated care initiatives and as these become more widespread and embedded in healthcare it is assumed by the plan that pressure on hospitals will be reduced.
The report summarises the Nuffield Trust’s findings on the impact of Age UK’s Personalised Integrated Care Programme (PICP) on hospital activity. The Nuffield Trust evaluated 40 initiatives.
